They are not "put to death". They are cast into the lake of fire. That is the second death. Not even sure when death and hell give up the dead to judgment that they're in physical bodies even then. As far as your last question, where is the rich man who wanted Lazarus to bring him a thimble of water, and where is the saint, of John's brethren, who conducted some of the tour in the book of Revelation?
