Many possibilities and nattering in tongues won't fix it.

Let's eliminate the two obvious ones you mentioned first. Is there a proper bird/animal gaurd on the termination?

Have you burned over a cord of wood since it has been swept and/or is there more than 1/4" of soot or creosote visible when shining a light up beyond the damper?

Chip & Dale? Isn't that a furniture style? If your chest of drawers got into the chimney it is a fair assumption you have a blockage.
