Ghost Writers

I have been reading a book written by someone who was a "ghost writer" for Billy Graham and a number of successful conservative ministers.

This has caused me to question what it really means when someone is a "ghost writer" for someone else. Does this mean they actually write the book for someone else? Would this not be deceptive for someone to publish a book as if they had written it, when in fact someone else wrote it?
