Need Help!

I am trying to find out where I can get the words and/or soundtrack to a song. I believe the title is Address Change, but I am not sure. It is a old song.

The chorus goes (I believe) like this: This is an address change notification, I'm moving to a much better place. There will be no tears and no dying, there will be no more pain. This is to tell you, of my address change.

Anyone know where I can find the words and/or soundtrack??? Any help will be greatly appreciated.
