Biblical church organization

Could someone expound the proper Biblical organization and government of a new covenant apostolic assembly (church)? Please include the following:

What are the necessary components or elements of a scriptural church?

How does a scriptural church come into existence?

What offices, if any, exist in a scriptural church? Please identify them, their names/titles, their roles/responsibilities, their qualifications, and how they are established/appointed/selected or otherwise come to be in a local assembly.

What are the purposes or missions of the church, in relation to the world, to its members, and to other churches?
Also, please be sure to address the following terms:
  • Pastor
  • Bishop
  • Elder
  • Presbytery
  • Overseer and oversight
  • Shepherd
  • Deacon
  • Minister
  • Steward
  • Servant
  • Evangelist
  • Prophet
  • Apostle
  • Teacher
  • Helps
  • Governments
  • Rule

Any insight appreciated, but please no just linking to someone else's article or just copy-pasting someone else's commentary or explanations. Also, please support everything with scriptural citations.

Church structure/purpose

Elders and Deacons oversee and administer the church (1 Tim 3)

Elders fulfill the roles of apostles, prophets, evangelists, pastors, and teachers (Ephesians 4:11) (Acts 15:2).

Elders edify the body of Christ to bring it to maturity (Ephesians 4)

Purpose of the church is the preach the gospel to every creature (Mark 16:15)

I'm interested, but will need to divest myself of other pursuits here. I am off the next two days. Hopefully I can get something together.

For now, I'm just going to build the skeleton, and hopefully have the time to come back in and fill in the rest as I go.

So, to start:

1 Corinthians 11:3,

3. But I would have you know, that the head of every man is Christ; and the head of the woman is the man; and the head of Christ is God.

The organizational structure of the church is as follows:

- God the Father, the Head of Jesus Christ
- Jesus Christ, the Head of the Church
- Body of Christ/Holy Nation/Royal Priesthood/Sons of God/Church
- The Twelve Apostles of the Lamb

Foundational Ministries:

- Apostolic Evangelists/Missionaries (e.g. Paul, Barnabas, Apollos)
- Prophets (e.g. Agabus, Silas, Timothy)

Local Church Expression (Including Governments and Helps)

- Local Assembly Elders/Shepherds/Overseers (must be apt to teach)
- Elder Women/Mothers of the Church (must instruct the young women)
- Deacons
- Mature saints
- Novice Converts

The Family Unit:

- Patriarch
- Matriarch
- Elders (e.g. Grandfathers/Grandmothers)
- Sons and Daughters

I am accustomed to a full time pastor and a volunteer church board.

The endless church cleaning problem is best fixed with a part time janitor.

When the budget permits, part time youth pastor or part time seniors pastor.
