All of my clients (I am probation officer) who are guilty of these charges were brought to justice by recordings. I would suggest that you buy a mini voice recorder and keep it with just in case he calls again.
Also, since a church is considered private property, if you tell him not to come there then he could be charged with trespassing. The key is without a shadow of a doubt you must tell him not to come on your property. I would send him an offical letter by certified mail -- telling him that because of his threats of violence he is no longer welcome.
Lord bless -- I will pray for your situation.

Of course we are praying, but here are the steps we are taking
Notifying all local law enforcement
Changing Locks on the Church (they had keys)
Doing a Restraining Order
Hiring a Security Guard for Sunday Services
Notified the Sheriff who lives across from me on my street
Instituting a partner policy, no leader is to be alone at the church, we operate two by two now on
Pulled out my Glock 17, now loaded and is in an safe yet, accessable place
Thanks to all for the wisdom and spirit behind your posts, this is something they don't teach in Seminary.

Dear Lord, Brother
What in the world did you do to cause a saint to react this way.
And does your action sound like Paul's instructions.
1 Cor 6:1
6:1 Dare any of you, having a matter against another, go to law before the unjust, and not before the saints?
KJV

Everything you're doing sounds good, except the gun.
