No, I do not believe that those exact words need to be spoken over the baptized person. Baptizing in the name of Jesus and baptizing in the name of the Father, Son, and Holy Spirit mean the same thing.

No. I believe that the baptism of the Holy Spirit is often or usually accompanied by tongues, but not always. A changed life is some good evidence, too.

No, these are man-made rules that can't be supported by Scripture (except probably short hair on men). If someone lives by these standards, that's great. But you can't condemn others for not following those standards.
