I've belonged only to churches that have business meetings of members with votes on board members and stuff, so that's all I know. Pastors were elected that way, usually with a nonbinding vote of confidence every few years. We could kick one out with a vote, though, with procedures in place. 2/3 majority, I think. (Once, we had one quit before we had the chance to try. He saw the writing on the wall!
