Mostly just Fundamental Baptist/Mennonite/Wesleyan churches, I think. From my experience, many denominations do not use Brother/Sister.

Re: Brother and Sister
I introduce myself as Mark. If someone feels the need to ADD brother to it I will politely tell them that I am not much on formalities and they can feel free to call me Mark. That usually works but for some reason this TRADITION has been so pressed onto people that there are many who still use brother.
most generally unless they are an elder I simply call people by their first name.
as for family I never preceded my flesh and blood brothers and sister names with their relationship to me when I speak to them so why should I feel like I have to with my spiritual brothers and sisters? now I have been 'talked to' by a few who think I am being disrespectful by NOT using brother and sister. BUT I politely tell them that I do not need to use a title to be respectful.

I've always made the point that I had never called my own biological sisters "sister" when talking to them, why should I do it for anyone else?
As far as offense goes, would not calling someone brother or sister cause them to stumble? If not, I respectfully ask them to shut up and let me live by MY convictions and not what they think I should be doing.

Re: Brother and Sister
I prefer to refer to the church family as Brother or Sister. Like Brother and Sister Cupples or Sis. Renda. I refer to all ministers as Brother (or Sister).
