My eyewitness account occured in 2011. Someone was baptized. The preacher was observing from the platform. There was no way the preacher could have asked the person how they wanted to be baptized. The preacher clearly invoked the name of Jesus in front of thousands of people. No one questioned the preacher.

While I have been gone from Columbus for quite a few years, As it pertains to your first question, YES, for nearly a year (1984-85) my wife and I had lunch with he and his wife many Sundays. I also taught a home Bible study to some of his inner circle back then, and while I will not state that is the reason for him re-baptizing many of his folks, (He also has a HUGE number of former UPC/other Apostolics in his church....and he has members of his extended family that are Apostolic preachers) some of his musicians and others were having Bible studies with him on the matter and he personally would send questions to me through them.
I told many Columbus area pastors they needed to quit hating on RP and get something happening in their churches. The reason people were leaving OP churches and going there was because something was happening. They have some of the best music, his preaching was pretty good back then (I personally think he has become too commercialized now and has lost some of his luster....JMO) and there were miracles and many other things happening.
Your eyewitness account is awesome! However, you and I both know that RP or whoever the keynote "preacher" happens to be, is NOT the one who makes the baptism proclamation every time. I assure you that just as in our OP churches what is said varies from preacher to preacher, and in WH's/RP's case how one is baptized is NOT an issue so baptism statements could vary.
My personal experiences with RP are more than 20 years ago and a lot may have changed since then.
