We take our kids to "family-friendly" restaurants. First of all, with 3 kids, a really nice restaurant can get very expensive very fast. For skinny little things, they can put away SO. MUCH. FOOD.  Secondly, for the reasons you mentioned, I wouldn't want to spoil anyone else's dinner. Parents tend to think everything their kids do or say is cute--and I'm included in that number. But I haven't deluded myself into thinking that everyone else shares my sentiments.
I do take issue with you comparing tolerating children to inhaling 2nd hand smoke, but I understand your point.
