He was confusing trangender with transsexual. Transsexuals who want sex reassignment surgery have to live as the opposite sex usually for one year and undergo extensive physiological evaluation before surgery.
Someone who identifies themselves as transgendered feels that their physical bodies don't match up to how they view themselves internally. Even though they may feel like a woman inside, they don't alter their physical bodies (male sex organ).
