Either elderly, over tired, drunk, on drugs, car not in driving safety condition etc..
Some willfully smoke knowing it will kill them.
There are 10's of Thousands each year that die behind the wheel of their cars.
Knowing that when they got behind the wheel someone out there would be driving inebriated.
